The Challenge
Northwell Health needed to enhance their patient care support application to better track post-admission care and reduce costly hospital readmissions. The existing system lacked automated care tracking features, staff found the technology difficult to use, and patient care management processes were inefficient. User satisfaction with the application was low (2/5 on quarterly surveys).
The Approach
Led architectural design and coordinated multiple development teams to deliver care tracking and automated solution features. Focused on user experience improvements to increase staff adoption. Implemented automated workflows for care follow-ups and early warning indicators. Worked closely with clinical staff to understand real-world usage patterns and pain points.
The Outcome
Achieved significant improvements in patient care management and user satisfaction. Readmission rates decreased measurably. The quarterly survey on ease of tech use and app fluidity improved from 2/5 to 4/5—a 100% improvement in user experience scores. Efficiency increased noticeably across care coordination workflows.
